Recovery and wellness are achievable but can be nearly impossible if an individual living with a mental illness, substance abuse or co-occurring disorder does not have a structured support system and a safe place during their recovery.
Y.A.N.A. House provides 24- hour supervision, personal care and therapeutic therapy, for clients who are struggling with behavioral health, substance abuse and co-occurring disorders in a safe, supportive, nondiscriminatory structured environment.
We do this through our comprehensive individualized treatment planning that involves the client and the care team management. We provided clients with an all-inclusive living arrangement while receiving daily therapy that cannot be found elsewhere. Clients are provided the tools necessary to create a life that is stable and cohesive by working with our expertly staff of Registered Nurses, Behavioral Health Professionals, Behavioral Health Techs, Social Workers and Caregivers.
Our clients will learn new and more effective coping skills, gain independency, learn to utilizes community resources and have a reduced probability of a crisis or relapse once completing treatment.
Y.A.N.A. House is dedicated to supporting the behavioral and mental health needs of Indigenous individuals by fostering cultural connectedness. Our approach integrates the expertise of Indigenous staff, access to traditional and spiritual healing practices, and a strong connection to the community, ensuring holistic and culturally appropriate care for those we serve.
YANA Transitional Housing is an individualized program tailored to young adults who are aging out of the foster care system or who need support in developing vital life skills for a sustainable and resilient lifestyle as determined by the participant with the help of G4C. We support participants in identify challenges, set SMART goals, access community resources, and build a connective network of support for on-going growth and stability. Our specialists work with the participants individually or in group settings to help provide psychoeducation surrounding healthy communication and relationships, how to navigate the workforce, education system, and how to obtain safe housing, all while acknowledging and honoring individual culture, traditions and worldviews. YANA provides up to 1 year of supportive services.
Provides access to safe housing and vital stepping-stones towards independence and self-sufficiency for transitioning into adulthood.
Our groups are designed to empower participants with skills and knowledge needed for self-sufficiency. As well as creating a sense of community and support from peers and mentors.
We provide on-site access and/or transportation to community partners for healthcare, behavioral health, housing, workforce development and other essential resources.
Our on-site donation support offers essential items that individuals need as they began to build their lives, ranging from hygiene products to furniture.
Our Life Skills & Personal Development program ensure each participant is learning the essential tools and skills to live successful, self-sufficient lives but also to learn how to create space for personal growth, through individual work and community connections.
Our Skilled case managers work closely with each participant to access their unique needs, develop personalized plans and provide ongoing support and advocacy.
